QT top-level menuitem can show only caption text or icon but not both

See attached example. If you set ImageIndex of top-level items to -1, then caption text shows. Now only icon shows.
GTK2 and Windows bindings show both text and icon.
I tried to fix it in r33428 but reverted because Zeljan wants to see the issue first.
Strange, there was another problem which I could reproduce consistently but can't any more: an icon (ImageIndex) defined for a top-level menuitem made icons from all sub-menus disappeared.
I thought I fixed that in r33428 but now it seems it made no difference. (?)
My changes can be seen in SVN history. I still believe that QT API call:
QMenuBar_addMenu(handle: QMenuBarH; icon: QIconH; title: PWideString)
is the right one but I didn't use it right.
